چکیده

CD3 is a subunit of the T-cell antigen receptor (TCR) complex required for its assembly and surface expression that also plays an important role in TCRmediatedsignal transduction.Wereporthere a patient with T B NK severe combined immunodeficiency (SCID) who was homozygous for a single C insertion following nucleotide 411 in exon 7 of the CD3 gene. The few T cells present contained no detectable CD3 protein, expressed low levels of cell surface CD3 , and were nonfunctional. CD4 CD8 CD3 low, CD4 CD8 CD3 low, and CD4 CD8 CD3 low cells were detected in the periphery, and the patient also exhibited an unusual population of CD56 CD16 NK cells with diminished cytolytic activity. Additional studies demonstrated that retrovirally transduced patient mutant CD3 cDNA failed to rescue assembly of nascent complete TCR complexes or surface TCR expression in CD3 deficient MA5.8 murine T-cell hybridoma cells. Nascent transduced mutant CD3 protein was also not detected in metabolically labeled MA5.8 cells, suggesting that it was unstable and rapidly degraded. Taken together, these findings provide the first demonstration that complete CD3 deficiency in humans can cause SCID by preventing normal TCR assembly and surface expression. CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Janus kinase 3 (JAK3) deficiency: clinical, immunologic, and molecular analyses of 10 patients and outcomes of stem cell transplantation

We found 10 individuals from 7 unrelated families among 170 severe combined immunodeficiency (SCID) patients who exhibited 9 different Janus kinase 3 (JAK3) mutations. These included 3 missense and 2 nonsense mutations, 1 insertion, and 3 deletions. With the exception of 1 individual with persistence of transplacentally transferred maternal lymphocytes, all infants presented with a T B NK pheno...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Human equivalent of the mouse Nude/SCID phenotype: long-term evaluation of immunologic reconstitution after bone marrow transplantation

Human Nude/SCID (severe combined immunodeficiency) is the first severe combined immunodeficiency caused by mutation of thewinged–helix–nude(WHN)gene, which is expressed in the thymus but not in the hematopoietic lineage. The disease is characterized by a T-cell defect, congenital alopecia, and nail dystrophy. A Nude/SCID patient who underwentbonemarrowtransplantationfrom the human leukocyte anti...

CLINICAL TRIALS AND OBSERVATIONS Human ICOS deficiency abrogates the germinal center reaction and provides a monogenic model for common variable immunodeficiency

The homozygous deletion of the inducible costimulator (ICOS), an activationinduced member of the CD28 family on T cells, causes an antibody deficiency syndrome in affected humans. The identification of a total of 9 ICOS-deficient patients revealed that this monogenic disease comprises the full clinical phenotype described for common variable immunodeficiency (CVID), including recurrent bacteria...

IL-21 Modulates Activation of NKT Cells in Patients with Stage IV Malignant Melanoma

Interleukin-21 (IL-21) is a common γ-chain cytokine produced by T helper and natural killer T (NKT) cells. It has been shown to regulate the response of various lymphocyte subsets including NK, NKT, T and B cells. Owing to its potent anti-tumor function in preclinical studies and its ability to induce cytotoxicity and interferon-γ (IFN-γ) production in NK and CD8 T cells, recombinant IL-21 (rIL...

Successful peripheral T-lymphocyte-directed gene transfer for a patient with severe combined immune deficiency caused by adenosine deaminase deficiency.

Ten patients with adenosine deaminase deficiency (ADA-) have been enrolled in gene therapy clinical trials since the first patient was treated in September 1990. We describe a Japanese ADA- severe combined immune deficiency (SCID) patient who has received periodic infusions of genetically modified autologous T lymphocytes transduced with the human ADA cDNA containing retroviral vector LASN. The...
